To provide the healthcare services you have requested, Strive Support Services needs to collect, use, and, where appropriate, disclose your personal and health information.

  • Prepare clinical reports and maintain your health record

  • Manage appointments, billing, and funding claims (including Medicare, NDIS, private health insurance, and DVA where applicable)

5. How do we protect your information?

Strive Support Services takes reasonable steps to protect your personal and health information from misuse, interference, loss, unauthorised access, modification, or disclosure.

Your information is stored securely using physical, electronic, and administrative safeguards. We only allow access to your information by authorised personnel who require it to provide your care or carry out their duties.

 

6. Online Activity.

You can visit our website without identifying yourself or providing personal information. We only collect personal information that you choose to provide, such as when you submit an enquiry or contact form.

Our website may also collect non-identifiable information, such as your IP address, browser type, pages visited, and the date and time of your visit. This information is used for statistical purposes to help us monitor and improve the performance of our website.
